Review: 3 Microneedling Treatment Cases
Three real patient presentations — acne scarring, enlarged pores, and hyperpigmentation — each with its own home-care and treatment plan. These cases illustrate clinical concepts commonly explored in structured microneedling training for healthcare professionals.

Microneedling is primarily applied to the face to address issues like acne, scars, dark spots, wrinkles, and large pores. It involves using small, sterilized needles to puncture the skin, promoting the formation of new collagen and skin tissue. The goal is to enhance skin texture, firmness, and tone through this cosmetic procedure.
Within this book, we analyze three cases well-suited for microneedling treatment, elucidating their respective treatment plans.
Clinical Solution: It is recommended to treat the rolling type of acne scars and acne-prone skin with a homecare routine that includes retinol 0.75 up to 1% (based on skin type) three nights a week and an oil-free moisturizer, stopped three days before starting microneedling treatment.
Between sessions, using Vitamin A serum and hyaluronic acid serum can be helpful, as well as taking oral supplements containing vitamin A, E, or antioxidant substances that stimulate collagen synthesis.
The interval between sessions should be three weeks, and the Dermapen product can be used during microneedling to accelerate the collagen production process as a complementary treatment. Additionally, it is recommended to undergo subcision one month before the first microneedling session to make the effects of microneedling much more effective.
Clinical Situation: Due to the visible open pores of the client, for a better effect of the treatment and longer durability, it needs a home routine 4 weeks before the treatment package and one week after the treatment package for at least six months. This pre-treatment and post-treatment home care routine will greatly improve the effectiveness of most microneedling sessions.
The pre-treatment home routine includes 10% exfoliation 4 nights a week and niacinamide, and three days before the first microneedling treatment session, this routine should be stopped and microneedling sessions with a depth of 0.75 to 1 mm should be performed until the skin turns pink.
Treatment intervals between sessions of two weeks are suggested, and it is better to use niacinamide between sessions. Taking oral supplements containing antioxidants and arginine is recommended.
One week after the last microneedling treatment session, a weekly home routine of exfoliating twice a week with oil-free sunscreen is required for up to six months.
Following microneedling, there is an increased requirement for moisturizing skin care products. Offer the client an oil-free moisturizer enriched with pore-tightening compounds, providing instructions on proper usage and frequency.
Clinical Situation: For effective hyperpigmentation treatment with microneedling, it is important to follow a skin care regimen before the treatment. Based on an individual's skin type, a one-month home care routine is recommended before starting the microneedling treatment package, along with the use of sunscreen and a rich moisturizer.
To prepare for the treatment, it is recommended to use Arbutin anti-spot compounds every night for four weeks before microneedling. For this case, the depth of the microneedling needle should not be more than 0.75 mm, and three passes are recommended in each session.
The best suggestion for this case is a treatment package of two sessions with an interval of one month. To maintain the treatment, it is recommended to use Vitamin C night cream and an oral supplement containing glutathione between microneedling sessions.
Continuing the anti-spot routine containing arbutin three nights a week and vitamin C three nights a week for up to three months helps to maintain the effectiveness of the treatment.
